Please be sure to complete the Client Intake Form prior to your first appointment.   

If you are interested in an Aromatherapy Massage, you will be asked to complete an Aromatherapy Intake Form.  This form will also be emailed to you; please return it prior to your appointment so I can set up the proper Aromatherapy session.  

Any essential oils you may be interested in purchasing, please feel free to talk with me, or visit my Young Living site. Please use my Member ID 12731187 if you sign up.
